I represent to you that:

  1. I have printed and read the Plan Disclosure Statement and agree to print and read the application and Participation Agreement (together, the "Agreements") prior to completing and mailing the application; and

  2. I will not alter the terms of the Agreements as originally set forth by you.

I acknowledge that any deviation from the Agreements' original terms shall be void, and I shall be bound by the terms of the original Agreements as set forth by you. I agree that Allianz Global Investors Distributors LLC and/or the South Dakota Investment Council may void the Agreements in the event there are reasonable grounds to believe the original Agreements have been altered.
