Use this form to update the beneficiary on your current CollegeAccess 529 Plan account.
UGMA/UTMA Account Transfer Form
Use this form to transfer assets from an account registered under the Uniform Gifts to Minors Act (UGMA) or the Uniform Transfers to Minors Act (UTMA) to an individual account in the name of the former minor beneficiary once the former minor has reached the age of majority for the state in which the account is registered.
Additional Investment Form
Use this form to make additional contributions to an existing account in the CollegeAccess 529 Plan. This form cannot be used to establish a new account.
Payroll Deduction Form
Use this form to authorize your employer to make periodic deductions from your paycheck, change how your payroll contributions are allocated amongst your beneficiaries, and stop payroll deductions to your current CollegeAccess 529 Plan account.
Change Form
Use this form to make changes to an existing CollegeAccess 529 Plan Account. Types of changes permitted include Bank Account Information Change, Investment Option Change, and Mailing Address Change.
Change of Ownership
Use this form to change ownership of an existing CollegeAccess 529 Plan account.
Name Change Authorization
Use this form to update your name on your current CollegeAccess 529 Plan account.
To submit a name change electronically, the Account Owner can click on the Electronic Name Change Authorization Form and complete the form online. Please note, documentation evidencing the name change must be uploaded.
Rollover Form
Use this form to rollover assets to the CollegeAccess 529 Plan from another Section 529 College Savings Account, a Coverdell Education Savings Account (formerly an Education IRA), or redeemed qualified U.S. Savings Bonds (see Plan Disclosure Statement for additional details).
Auto-Exchange Form
Use this form to establish the auto-exchange option on your CollegeAccess 529 Plan account. This option allows you to regularly exchange units from one CollegeAccess 529 Portfolio to another within the same class. (Please note: exchanges between 529 Plan accounts with different registrations are not allowed.)
Withdrawal Request Form
Use this form to request a full or partial withdrawal from your CollegeAccess 529 Plan account.
Broker Dealer Change Form

Please use these instructions to invest via Federal Funds Wire. Transfer funds directly to your CollegeAccess 529 Plan account. Your bank may charge you a fee for sending a wire to the plan. Include the name of portfolio, account owner name and account number (do not include the portfolio number).
